- You are between the age of 18-74.
- You have chronic disease that requires anticoagulation.
- You are NOT requesting a new prescription.

The ability to discuss your illness and current treatment.
What to expect from your Virtual Visit
Evaluation of anticoagulation with possible labs ordered and refill of current medications.

- Coumadin/warfarin
- Direct Oral AntiCoagulants (DOAC’s)
